Why is this contact form not working? (uses php-mailer)?

When I try to access a href="http://www.promaxcontractors.com/scripts/new_code.php" rel="nofollow"http://www.promaxcontractors.com/scripts…/a , the response that I get (as shown by Wireshark) is "HTTP/1.1 500 Internal Server Error" and Wireshark reports an error in decompressing the body fo the response: "Content-encoded entity body (gzip): 20 bytes [Error: Decompression failed]". Check your web server logs to see what error occurred (e.g. my attempt at Date: Tue, 19 Jan 2010 01:18:55 GMT) & a href="http://www.getnetgoing.com/Internet-Explorer-The-website-cannot-display-the-page.html" rel="nofollow"http://www.getnetgoing.com/Internet-Expl…/a explains why IE is showing that error (due to the 500 internal server error).
